The Miscarriage Conversation Nobody Is Having With Men

A woman loses a pregnancy.

And almost instantly, the questions begin. “Was it her stress?” “Is something wrong with her uterus?” “Did she do something wrong?”

The guilt lands on her. The blame lands on her. The silence lands on her.

This has gone on for far too long.

It is time to have a conversation that Indian families, doctors, and couples have been avoiding for years. Miscarriage is not a woman’s failure. It is a shared biological outcome. And science has been telling us this for a long time.

The Number Nobody Talks About

Pregnancy is 50% sperm.

That is not an opinion. That is basic biology.

Half the genetic material of every embryo comes from the man’s sperm. So the question is: if pregnancy is a 50-50 contribution, why does 100% of the blame land on the woman when something goes wrong?

With 50% of infertility being an issue with the male partner, sperm quality plays a major role in miscarriage and recurrent miscarriage. Yet most couples going through miscarriage are never told this.

  • The man never gets tested beyond a basic semen analysis.
  • The woman goes through cycle after cycle of investigation, procedures, and heartbreak, while her partner’s health remains completely unexamined.

That needs to change.

What Sperm Actually Does (That Most People Don’t Know)

Most people think sperm has one job: reach the egg and fertilize it.

But that is only part of the story.

Sperm quality is determined by several key factors, including count, motility, morphology, and genetic integrity. High-quality sperm increases the chances of successful fertilization and healthy embryo development.

The piece that gets almost no attention? DNA integrity.

Sperm does not just deliver genetic material to the egg. It delivers the quality of that genetic material.

And when that DNA is damaged or fragmented, the consequences can be devastating, even when everything else about the sperm looks perfectly fine on paper.

The Real Villain: Sperm DNA Fragmentation

Here is what sperm DNA fragmentation means in plain English.

Think of your DNA as a long thread.

  • In healthy sperm, that thread is intact and unbroken.
  • In damaged sperm, that thread has cuts and breaks in it, called fragmentation.
  • When a fragmented sperm fertilizes an egg, the embryo it creates carries those breaks in its genetic code.

Poor sperm quality may contribute to miscarriages through a range of mechanisms including genetic abnormalities.

Aberrations in sperm DNA can lead to issues with embryo development, increasing the likelihood of a miscarriage.

Research suggests that the presence of DNA damage in sperm can more than double the risk of miscarriage.

More than double. Let that sink in.

This is not a fringe theory. This is peer-reviewed science. And yet the DNA fragmentation test is rarely included in a standard fertility workup for men.

“But His Report Was Normal.”

This is one of the most frustrating sentences in fertility medicine.

A standard semen analysis checks three things: sperm count, motility (how well they swim), and morphology (their shape). These are useful parameters. But they tell you nothing about DNA integrity.

A man can have:

  • A high sperm count
  • Good motility
  • Normal morphology

And still have severely damaged sperm DNA.

Poor sperm health, including reduced count, motility, morphology, and DNA integrity, may increase the risk of implantation failure or early pregnancy loss, particularly in cases of recurrent miscarriage.

This is exactly why couples who have “normal” semen reports keep experiencing unexplained miscarriages. The right test was never done. And the right conversation was never had.

At Team Luke, we have seen this pattern time and again. A man walks in with a clean semen analysis report, confident that fertility is “her problem.”

But when we look deeper at his lifestyle, his stress levels, his sleep, his diet, his habits, a very different picture emerges. Foundational Medicine asks us to look at the whole person, not just the report.

What Is Actually Damaging Sperm DNA?

This is where the conversation gets very practical. Because the causes of sperm DNA damage are mostly lifestyle-driven. Which means they are mostly preventable and reversible.

Here is what the science says about causes of sperm DNA damage:

FactorHow It Damages Sperm
SmokingGenerates oxidative stress, directly damaging sperm DNA
AlcoholDisrupts hormone balance and sperm production
Poor dietDeficiencies in zinc, folate, and antioxidants impair sperm quality
Chronic stressElevates cortisol, which suppresses testosterone and sperm health
ObesityIncreases scrotal temperature and estrogen levels, harming sperm
Environmental pollutionToxins and heavy metals cause DNA fragmentation
Sleep deprivationDisrupts hormonal rhythms essential for sperm production

Instead of looking only at reports, the foundational approach focuses on rebuilding the body’s foundations: nutrition, sleep, movement, stress management, and daily habits that influence fertility and sperm quality.

In short: your everyday choices are either building healthy sperm or destroying it. There is no neutral.

Age Is Not Just a “Women’s Problem”

Society has spent decades telling women that their biological clock is ticking. Rarely does anyone say the same thing to men.

But here is the truth about paternal age and miscarriage:

Although men can produce sperm until their 70s and even beyond, the quality of the sperm deteriorates over time. Age-related changes and male fertility decline begin in the mid-30s in males. These changes can make conception difficult and increase the risk of miscarriage and other prenatal disorders.

  • One of the most critical changes observed in males as they age is the increase in DNA fragmentation or damage in the DNA of the sperm.
  • Damaged DNA significantly impacts the quality of the embryo formed, further leading to heightened risks of miscarriage and genetic abnormalities.

This does not mean men over 35 cannot father healthy children. Of course they can. But it does mean that age-related changes in sperm are real, measurable, and relevant to miscarriage risk. Ignoring them is not just unscientific. It is unfair to the women carrying the entire burden of investigation.

Oxidative Stress: The Invisible Sperm Killer

Here is a term worth knowing: oxidative stress.

Oxidative stress happens when there are too many free radicals in the body and not enough antioxidants to neutralize them. It is linked to poor diet, smoking, pollution, stress, and poor sleep. And it is one of the leading causes of sperm DNA damage.

The solution is not complicated.

  • Antioxidant-rich foods, reduced smoking and alcohol, better sleep, managed stress, and movement. These are not miracle cures. They are the basics of what Team Luke calls Foundational Medicine, the non-negotiables that your body needs to function and heal.

When we rebuild these foundations, the body’s ability to produce healthy, high-integrity sperm improves dramatically. We have seen it happen.

The Emotional Weight Women Are Carrying Alone

For every statistic in this blog, there is a woman who has sat in a doctor’s office, in a hospital bathroom, or in her own bed and asked herself: “What did I do wrong?”

The answer, in so many cases, is nothing.

Miscarriage is multi-factorial. Chromosomal abnormalities, uterine factors, hormonal imbalances, immune responses, and yes, sperm DNA quality, all play a role. It is rarely one thing. And it is almost never the woman’s fault alone.

But the way we talk about miscarriage in India ensures she carries the guilt by herself. Her diet is questioned. Her stress is blamed. Her body is put on trial.

Meanwhile, his lifestyle habits, his sleep patterns, his smoking or drinking, his untested sperm DNA, remain entirely off the table.

This is not just medically incorrect. It is deeply unjust.

What Needs to Change

Here is what a more fair, more intelligent, and more effective approach to miscarriage and recurrent pregnancy loss looks like:

Step 1: Test both partners from the start. Not just a basic semen analysis. Ask specifically about a Sperm DNA Fragmentation Index Test (DFI).

Step 2: Look at lifestyle, not just lab reports. A “normal” report does not mean optimal. Sleep, stress, diet, smoking, alcohol, and weight all matter enormously for male fertility.

Step 3: Stop gendering the conversation. Fertility is not a women’s health issue. It is a couple’s health issue. Both partners need to be equally involved, equally investigated, and equally accountable.

Step 4: Address the emotional side. You don’t have to figure it out alone. Couples navigating recurrent miscarriage need support, right information, and guidance rooted in root-cause health, not just symptom management.

The Foundational Medicine View

At Team Luke, we believe that health begins at the foundation.

When couples come to us after recurrent miscarriage, we do not just look at the woman’s hormones or uterine lining. We look at both partners: their nutrition, their sleep quality, their stress load, their toxic exposures, their emotional health, and yes, their sperm DNA quality.

Fertility health is not a monthly event. It is built day by day, through the choices you make at the dinner table, in the bedroom, in the office, and in your mind.

The good news? Most lifestyle-driven causes of poor sperm quality are reversible. With the right guidance, the right nutrition, and the right foundations in place, men can meaningfully improve their sperm health and their contribution to a successful pregnancy.

This is not about blame. It never was.

It is about awareness. It is about shared responsibility. And it is about finally giving women the relief of knowing that this was never entirely theirs to carry.
